Little League International and Minisink Valley Little League have several forms that are required to be completed depending on how you register your children and family. Failure to comply with submission of the required forms will cause a delay in placing your child on a team.

New Members (Families) of Minisink Valley Little League the following forms are required:
1. Birth Certificates of all NEWLY registered children.
2. 2 Proofs of Residency, ideal proofs are (must show physical address PO Box will NOT be accepted.

  • a. Copy of Driver’s license
  • b. Utility Bill
  • c. Property Tax Bill
  • d. Copy of Homeowner’s or Auto Insurance.

3. Completed  Medical Release form   for each Child.
4. Completed NEW Volunteer Form   and copy of your Driver’s License for perspective Coaches and Volunteers.

Returning Members (Families) ADDING (registering) a new CHILD (did not play in 2016):

1. Birth Certificates of all NEWLY registered children. .
2. Completed Medical Release Form for each Child.
3. Completed Returning Volunteer Form  is requited for perspective Coaches and Volunteers.

Return Member (Families)Re-registering children from the 2016 season.
1. Completed  Medical Release Form for each Child.
2. Completed Returning Volunteer Form is requited for perspective Coaches and Volunteers.
